In a series of four 2022 focus groups with staff from nine state IDD agencies and the National Association of State Directors of Developmental Disabilities Services (NASDDDS), this study explored state experiences with implementing the 2014 Medicaid Home and Community Based Services (HCBS) rule. Participants described the process of shifting HCBS programs to address provisions of the rule, challenges associated with changing federal guidance, ways that the provisions of the rule were embedded into state policy, changes in service delivery, and challenges and opportunities associated with the COVID-19 public health emergency (PHE). Despite serious challenges, participants identified an array of improvements to HCBS as they implemented the HCBS rule.
